(DURANGO, COLO.) Recently, Tamra Lavengood, Clinical Performance Coordinator, Mercy Family Medicine – Centura Health Physician Group, was recognized at the national meeting of Comprehensive Primary Care Plus (CPC+) in Baltimore for her exceptional contributions to improve and innovate primary care. According to CPC+, “this year's winners went above and beyond by engaging with their CPC+ colleagues around the country to share their successes, lessons learned, and best practices.” This year’s meeting was attended by more than 1,000 people.
“This recognition is not about me, but about all of us making primary care primary,” Tamra explained.
CPC+ began in 2017 with 2,876 primary care practices across the nation; 18 regions (states); 14,810 primary care providers; and over 2 million Medicare patients. CPC+ provides multi-payer support, that enables primary care practices to build the infrastructure needed to provide whole person care. An initiative of the Center for Medicare and Medicaid Innovation, CPC+ is a national advanced primary care medical home model that aims to build more comprehensive care within primary care clinics.
Tamra has been involved with CPC for the last 7 years beginning with CPC Classic and now CPC+.
Because of its involvement in CPC, Mercy Family Medicine – Centura Health Physician Group has been able to:
- Hire RN Care Coordinators who care manage the most complex patients.
- Hire behavioral health providers who are imbedded in MFM clinics and available during office visits.
- Have diabetic educators in clinics for teaching and support.
- Follow up on all Emergency Department visits and hospitalizations.
- Improve health outcomes and reduce patient’s costs by avoiding hospitalizations.
- Demonstrate improved quality and reduce costs.
“The amazing work that Mercy primary care has done, gives me the opportunity to speak both locally and nationally about their successes,” Tamra said. “I have spoken at HIMSS Conferences; Hospital Associations; Scottsdale Institute; CPC Learning Collaboratives both regionally and nationally; SIM Learning Collaboratives; faculty for the Maryland Primary Care Program; locally for CHAC; and several other engagements.”
“I believe that primary care is the backbone for health care reform, and Mercy Family Medicine is demonstrating this,” Tamra added.
