Colorado Health Neighborhoods was formed in 2011 and is dedicated to delivering quality, compassionate care. We offer convenient access to care, user-friendly tools and resources to manage health for a lower cost and improved experience, and a personalized care approach so members can meet their health and wellness goals.

Colorado Health Neighborhoods contracts with accountable care organizations (ACOs) and insurance plans. An ACO is a group of physicians, hospitals and other health care providers who work together to give coordinated, quality care to patients while working to decrease health care costs and enhance the overall member and provider experience.
