About us

We are excited to announce the creation of Mountain Health Network (MHN), a clinically integrated network dedicated to improving outcomes and making care more streamlined and affordable. Serving patients and providers in Utah and Colorado, MHN is one of the largest physician-led clinically integrated networks in the region with a focus on improving health outcomes for our communities through adoption of evidence-based practice, investment in population health infrastructure, and, most important, robust physician engagement.

MHN is proud to introduce its leadership team! These individuals guide multiple teams of hardworking professionals to deliver high-quality information and services to you.

Our Leaders

Cindy is a 28-plus-year healthcare industry veteran who is leading the initiative to develop Mountain Health Network in Colorado, Utah, and Kansas. Before her role as Regional VP of Population Health, Cindy worked for various hospitals and health systems, with her early career focused on managed care. However, her main passion is population health and building clinically integrated networks, of which she has a bounty of experience.

While at Southern Illinois Healthcare, Cindy developed the Quality Health Network (QHN) and served as its first Executive Director. She then moved on to Colorado, where starting in 2020 she served as the SCL Health Population Health leader over all regions and the Senior Administrator for the Colorado Quality Care Network (CQCN). In 2023, she became part of the Intermountain Health joint venture with UCHealth, where she served as the Director of Payer Contracting and Network Development until she joined the Mountain Region in March 2024.

Dr. Eileen Gallagher is a Family Medicine physician who has been in healthcare for over 20 years. She grew up in the beautiful redwoods of Northern California, where she developed a passion for the outdoors. She went to the University of California, Davis, majoring in Nutrition Science, then spent a post-graduate year at the University of California, San Francisco completing a dietetics internship. She then attended the University of California, Irvine for medical school and completed residency in Portland, Oregon at Oregon Health Sciences University (OHSU) in Family Medicine.  

She moved to Colorado in 2004, practicing full-spectrum Family Medicine in a multispecialty group practice for over 15 years, including OB, NICU care and both routine and preventive care. Dr. Gallagher joined CommonSpirit in 2018 as the Medical Director of Risk Adjustment and has been the Physician Director for the network since 2020. In that role, she supports and manages the clinical teams including Population Health, Quality, Risk Adjustment, Data & Analytics, and Care Management. She also served as the interim president for CHN this past year during our significant transition.

Dr. Gallagher was initially drawn to population health due to the ability to impact her community by advocating for patients and clinicians in our complex medical delivery system. This is more true today as our system has become much more complex in the past five years with the pandemic and world changes.

When not working, Dr. Gallagher enjoys spending time with her two teenage children, outdoor activities, reading, and cooking.

Adam is an accomplished healthcare executive with 20 years of experience in multiple value-based payment and care delivery models, corporate development, strategy, operations, and leading new ventures into new markets. His executive experience includes various leadership roles with renowned large, value-based health system, Kaiser Permanente, as well as multiple accomplishments leading a small digital health company and expanding into new markets.

Adam is a passionate leader looking to make meaningful transformations in our healthcare system by working with and supporting forward-thinking provider groups to deliver the best possible care to their patients. 

When he is not working, Adam enjoys taking part in his kid’s activities, coaching, and getting out for a bike ride or a day on the slopes.

Emily is a newer addition to CommonSpirit and Mountain Health Network (MHN), joining us in August 2024. She started her healthcare career in 2013 at Catholic Health Initiatives, supporting their PMO and Data Governance team. In 2016 she transitioned to SCL Health as a Senior Project Manager in their Enterprise Program Management Office (EPMO), focused primarily on Epic implementations. In August 2020, Emily was assigned as the project manager for the SCL Health Front Range Clinically Integrated Network initiative; it was in this role she discovered her interest in population health and CINs. She spent the next 2.5 years supporting the development of Colorado Quality Care Network (CQCN). In March 2023, she accepted the position of Operations Manager, Value-Based Programs, a role she held until the opportunity to work on MHN was presented to her. 

Emily lives in Denver with her husband, Andrew, and their three dogs. She enjoys spending her free time cooking and world traveling with her husband.

Jennifer has been with CommonSpirit since May 2023, with her primary role as the population health operations manager, and building relationships with practices in the Utah market. She began her career in hospitals as a Registered Respiratory Therapist and was in that role for over 10 years. Her role then evolved into a preceptor with respiratory students, where she realized she had a passion for working with students.

Jennifer then served in the role of Clinical Director of Education for Steven Henager’s College Respiratory Therapy Program and eventually became the Associate Dean. In 2021, she accepted a position with Steward Health, working in population health management until their merger with CommonSpirit in 2023.

Rebecca has been the Provider Relations Lead for Colorado Health Neighborhoods since March 2020 and will continue to do so as part of MHN. Her primary role is to foster strong collaborative relationships with healthcare physicians, ensure effective communication and coordination, and drive initiatives to enhance the quality of patient care.

Before March 2020, she was the Vice President of Operations at Healogics for 10 years, overseeing 15 advanced wound care clinics across five states. In that role she cultivated her experience in delivery of high-quality patient care, maintaining high standards using established Clinical Practice Guidelines, and ensuring compliance with healthcare regulations and safety standards.

Rebecca lives in Broomfield, Colorado with her husband, where she enjoys spending time with her two grown daughters and three grandchildren, and plenty of time outdoors.

As the Quality and Risk Director, Lisa is tasked with the development, implementation and oversight of our Mountain Region Value Hub Quality and Risk department to ensure the delivery of high quality, coordinated and efficient care for patients covered under Common Spirit Health's value-based contracts. She oversees all of our Quality and Risk staff and departments and will work with other CommonSpirit Health leadership teams across our other Value Hubs and the enterprise.

Lisa has previous roles within CommonSpirit over the past eight years serving in many Population Health roles beginning with the Population Health Coordinator and moving up through the management ranks to our Manager of Population Health. Initially she helped develop our Comprehensive Primary Care Plus program then expanded many of our additional Colorado programs including MAC, ISP, BH 1302, APM1, APM2 and  the CMS' Primary Care First Program. Most recently as the Manager of Population Health, she oversaw our team of Outpatient Care Coordinators and led our Population Health Team to meet payer contract needs and help physicians and APPs meet quality goals.

Prior to working at CommonSpirit, Lisa worked in the insurance and financial industry as a quality and risk manager and as a quality and practice optimization coach with independent practices around the US. She has her Masters of Healthcare Administration (MHA) from Colorado State University and her undergraduate degree in Business and Communications from Stephen F Austin State University and holds her Lean Six Sigma Certification.

In her free time, she enjoys spending time with her three children and reading, hiking, and playing golf.  

Janet joined Centura in 2019 as Manager of Regional Ambulatory Care Coordination. She has over 40 years experience as a registered nurse working acute care, specialty clinic, payer based care management and independent population health management.

In her previous leadership roles she has dedicated the last 16 years to implementing and driving performance in the areas of transitional care management, complex care management and disease management.  

In her current role with CommonSpirit Health she leads initiatives to provide holistic comprehensive care to patients resulting in decreased  readmissions, avoidable ED use while empowering patients in proactive self management. Patients are assessed and connected with community and behavioral health resources as identified.

Janet lives in Littleton with her husband Bob. She enjoys spending time on walks/hikes with her husband along with music, travel, crocheting and reading.

We are a Clinically Integrated Network

Clinical integration is defined by an active and ongoing program to evaluate and modify practice patterns by the network’s physician participants, thus creating a high degree of interdependence and cooperation among the physicians to control costs and ensure quality:

  • A coordinated approach to clinical and quality performance, improvement programs, care management and navigation, use of evidence-based guidelines, patient engagement and satisfaction, and performance reporting
  • Expectations and requirements for network participating providers that facilitate optimal performance

Membership Benefits

MHN physicians and advanced practice providers (APPs) are at the core of everything MHN does. By helping participants deliver high-quality care, the network is better able to deliver the best care possible to the communities served. MHN is constantly evaluating and developing new ways to support patients and providers to achieve improved outcomes, greater efficiency and/or lower cost.

Continuing Care: MHN’s Continuing Care Program is designed to enhance the quality and efficacy of patient management, improve patient experience, and promote continuity of care and cost effectiveness through integrating the functions of care management. MHN helps patients transition from the acute setting back to primary care and connects patients with community resources and ongoing care management support.

The Quality, Risk, and Population Health services are designed to objectively and systematically evaluate the quality, appropriateness and outcome of care and services delivered to patients, and to assist practices with closing clinical gaps related to quality HEDIS measures. MHN provides member practices with the following support:

  • Collect, aggregate, analyze, and display data for understanding of current performance and the identification of opportunities for improvement
  • Training and education interventions for improvement are tailored at the practice level for physicians and care teams. 
  • Quality coordinators will work closely with your practice when patients are identified as having a care gap, such as needing a mammogram, colon cancer screening or diabetes testing
